First Visit Guide & What to Bring
When you come for your appointments, please bring the following:
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
- List of medications (if any)
To cancel an appointment
Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
Insurance Information
We accept most major insurance plans.
Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Fees & Payments
Our Office has a cash-only policy for Co-Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals during our normal business hours, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
